You will need to print the appropriate form and complete all relevant sections. Then date, sign and mail it to the address noted below. If you do not see a form suitable for your request please contact our Inforce Services department at 1 (800) 832-7783, Monday—Friday, 9:00AM to 6:00PM Eastern Time and one of our customer service representatives will be happy to assist you.

Life Insurance Policies and Annuity Contracts
- Change the Name on my life insurance policy or annuity contract.
- Change the Owner on my life insurance policy or annuity contract.
- Change the Beneficiary on my life insurance policy or annuity contract.
- Establish a monthly electronic payment for my life insurance policy or annuity contract.

- Establish a Systematic Withdrawal from my annuity contract.
- Change the Subaccount Allocations for my:
- FIRST CHOICE—Variable Deferred Annuity
- Tax Tamer® Variable Deferred Annuity
- Establish or cancel the Systematic Transfer or Auto-Rebalancing options for my:
- FIRST CHOICE—Variable Deferred Annuity
