Universal Forms
Family of First Investors Funds Application for Broker/Dealers (FIAPP-BD) — The Family of First Investors Funds Application is used with First Investors Mutual Fund Account Instructions to establish new First Investors Fund accounts.
First Investors Mutual Fund Account Instructions Form (FIMFA-BD) — The First Investors Mutual Fund Account Instructions Form is used with the Family of First Investors Funds Application to establish new First Investors Fund accounts.
Affidavit of Correct Name or Name Change (AFRM015) — The Affidavit of Correct Name or Name Change form is used to correct a spelling error found in a First Investors Fund account registration or to change the name on a First Investors Fund account when a customer has legally changed their name.
Affidavit of Domicile (SLTR297) — The Affidavit of Domicile form is used to confirm the state of residence for a deceased person.
Electronic Funds Transfer Service (EFT) Application (EFTAPP) — The Electronic Funds Transfer Service Application is used to request or change an electronic investment via Automated Clearing House (“ACH”) into a First Investors Fund account.
IRS Form W-9 — The Form W-9 is used to certify a taxpayer tax identification number.
Acceptance of Class B Share Purchase Orders (BDCLASSB) — The Acceptance of Class B Share Purchase Orders form is required when purchasing more than $100,000 or more in a First Investors Fund Class B shares.
First Investors Agent's Certification Form (FIAC) — The First Investors Agent's Certification Form is used to establish an agent's authority when acting as Executor, Administrator, Guardian or Conservator. This form allows any one authorized agent to act alone.
Cash Management Fund Payment Form (MMPF-BD) — Use to redeem Cash Management Fund shares and invest proceeds in a mutual fund or FIL variable life insurance or annuity product.
Non-Retirement Forms
Certificate of Authority for Broker Dealers (BDCOA) — The Certificate of Authority form is used to identify the authorized individuals who are allowed to act on behalf of Corporations, Partnerships, Trusts, Sole Proprietorships or other Institutional Accounts.
Transfer on Death (TOD) Registration Request Form (TOD001) — The Transfer on Death (TOD) Registration Request Form is used to add or change TOD instructions on a First Investors fund account.
Certificate of Entitlement for TOD Accounts (TOD002) — The Certificate of Entitlement for TOD Accounts form is used by a beneficiary to certify that the account owner is deceased and to instruct the transfer or liquidation of a First Investors Fund account.
Education Savings Account (ESA) Distribution Request (ESADISTR) — The Education Savings Account (ESA) Distribution Request form is used to request a distribution from a First Investors Fund ESA account.
Joint Account Supplement to FI Funds Application (MAASUPBD) — The Joint Account Supplement form is used when opening a jointly registered account with more than two owners or if the second owner's residential address is different from the first owner.
Mutual Fund Privilege Application (MMPRIV) — The Mutual Fund Privilege Application is used to request additional privileges after a First Investors Fund account has been established.
Transfer Form (Change of Ownership) (MAATR) — This Transfer Form is used for transfers to an Individual, Joint or Uniform Transfer to Minors Act (UTMA) account . It is not to be used for First Investors Retirement Accounts.
Retirement Forms
IRA Distribution Request (MISC031) — The IRA Distribution Request form is used to request funds from a First Investors Traditional IRA, SEP-IRA, SARSEP, or SIMPLE-IRA Fund account.
TDI 403(b) 7 Distribution Request (403B7DR) — The TDI 403(b) Distribution Request form is used to request a distribution from a First Investors Fund TDI 403(b) account.
Roth IRA Distribution Request (ROTHDIS) — The Roth IRA Distribution Request form is used to request a distribution from a First Investors Fund Roth IRA account.
IRA/403(b) Beneficiary Designation Form (BENEFORM) — The IRA/403(b) Beneficiary Designation Form is used to change or revoke the beneficiaries listed on a First Investors IRA (Roth, Traditional, SIMPLE, SEP or SARSEP) or 403(b).
Required Minimum Distribution (RMD) Form for IRAs (RMDF-IRA) — The Required Minimum Distribution Form for IRAs is used to request a required distribution from a First Investors Traditional IRA, SEP-IRA, SARSEP, or SIMPLE-IRA Fund account.
Required Minimum Distribution Form (RMD) for 403(b) Accounts (RMDF-TDI) — The Required Minimum Distribution Form for 403(b) Accounts is used to request a required distribution from a First Investors Fund 403(b) custodial account.
Form W-4p 2010 — The Form W-4p 2010 is used to instruct tax withholding.
First Investors IRA to First Investors Roth IRA Conversion Form (ROTH001) — The First Investors IRA to First Investors Roth IRA Conversion Form is used to transfer assets from a First Investors Traditional IRA, SEP-IRA, SARSEP, or SIMPLE-IRA Fund account to a First Investors Fund Roth IRA account.
Individual Retirement Account (IRA) Application (BDIRAAPP) — The First Investors Individual Retirement Account (IRA) Application Booklet contains the agreement, application, and additional forms needed to establish a First Investors Traditional IRA, Conduit IRA, Roth IRA, Inherited Traditional IRA, and Inherited Roth IRA.
Simple IRA - SEP IRA - SARSEP IRA Booklet (BDS-IRA) — The First Investors Simple IRA, SEP IRA, SARSEP IRA Application Booklet contains the agreement, application and additional forms needed to establish a First Investors Simple IRA, SEP IRA, SARSEP IRA Account.
Education Savings Account (ESA) Application (BDESAAPP) — The First Investors Education Savings Account (ESA) Application Booklet contains the agreement, application and additional forms needed to establish a First Investors Education Savings Account.
403(b)(7) Authorization Form (403AUTH) — For an Employer or Third Party Administrator on behalf of an Employer to state that the Receiving Provider is approved and/or has an Information Sharing Agreement in place.
